Why we exist...
Individuals without permanent housing and those who have experienced historical discrimination and marginalization face significant healthcare disparities. These challenges are compounded by poverty, which introduces barriers such as unaffordable co-pays, lack of paid sick leave, and inadequate transportation.
Rooted Healthcare addresses these needs by offering affordable, accessible, and comprehensive care within the communities where people live, worship, and work.
We collaborate with local shelters, faith-based organizations, food pantries, and other trusted neighborhood locations to deliver care to those facing barriers to traditional healthcare services. Our services include patient registration for insurance, care coordination, wellness and prevention screenings, care for acute illnesses and injuries, chronic condition management, and referrals to external healthcare and social services.
